Registration forms can be mailed, emailed, or faxed to
the Cystinosis Foundation.
Registration forms must be received as soon as
possible and no later than June 1, 2004

Transportation
Activities for Children and Teens
Many different activities are planned for children of all ages. Activities will be available during conference sessions and will provide an opportunity for youth to have fun and meet new friends while their parents are in the conference sessions.
Child Care
Complimentary child care will be available during conference hours. Parents are responsible for their children during the lunch break. Pre-registration and signed permission statements are required. Children must be checked in/out by parents each day, and may not leave child care area without a parent.
All medications must be administered by the parent.
